Program Fees and Rules:
– One-time setup charge – $100
– Consolidated Bill Service Fee – $15/month/group
– Fees apply for case modification and plan changes
– Voluntary Package must be selected
– All employees must make active decision on each product
– Agreement for Premium Remitter must be executed with chosen carriers.
– Benefit administration system built in 7-10 business days
